Usually yes. The access, the setup and the travel happen once, so the price per light falls as the number rises. If you are going to do it eventually, doing it in one visit is the cheaper way.

Almost never. Most halogens are a 90mm cut-out and the LEDs go straight into the same hole. Where a ceiling has an odd size we match it rather than enlarging it, which matters in older and heritage ceilings.
They come out. Leaving them in the ceiling is the shortcut that gets taken and it is both a heat problem and a waste — an LED does not need one. The old fittings and transformers leave with us.
Ten lights is a morning. The old ones come out, the new ones go in, and the room is left as it was found. Every job gets a certificate of compliance.
